The new european guidelines have included the NYHA class 2 in the spectrum of indications for ventricular resynchronization therapy (CRT) , as well as patients suffering from ischemic cardiac insufficiency class 1, with an LVEF ≤30% and QRS ≥130 mseg. For patients with NYHA class 3-4, CRT in indicated whenever the QRS is 120 mseg or in those class 2 patients with QRS of 150 mseg. Clearly, the patient must have an LVEF ≤35%, sinus rhythm and very best medical treatment.
